Mid systolic septal deceleration in hypertrophic cardiomyopathy: clinical value and insights into the pathophysiology of outflow tract obstruction by tissue Doppler echocardiography.

چکیده
H ypertrophic cardiomyopathy (HCM) is an inherited cardiac disorder characterised by abnormal left ventricular (LV) hypertrophy in the absence of severe aortic stenosis and hypertension. The presence of a dynamic gradient in the left ventricular outflow tract (LVOT) above 30 mm Hg at rest is associated with an increased mortality. Surgical myectomy or transcoronary ablation of septal hypertrophy (TASH) is indicated in patients with persistent symptoms and gradients . 50 mm Hg under resting or provoked conditions, despite medical treatment. We recently described a characteristic mid systolic septal deceleration (MSSD) pattern in the velocity trace of the basal septum obtained by tissue Doppler imaging (TDI), but the diagnostic value of the MSSD pattern in a larger patient population is still unknown.

Evaluation of left ventricular outflow tract obstruction with 4D phase contrast in patients with hypertrophic cardiomyopathy
Background Patients with hypertrophic cardiomyopathy (HCM) develop left ventricular outflow tract (LVOT) obstruction due to combination of asymmetric septal hypertrophy and systolic anterior motion of the mitral leaflet. Degree of LVOT obstruction is an important predictor in patient symptoms and clinical outcome. LVOT obstruction is currently assessed by Doppler echocardiography (echo) using p...
متن کامل[Left ventricular remodeling in patients with hypertrophic obstructive cardiomyopathy treated with percutaneous alcohol septal ablation: an echocardiographic study].
INTRODUCTION In patients with hypertrophic obstructive cardiomyopathy, obstruction in the left ventricular outflow tract may generate more hypertrophy. Our aim was to evaluate the impact of reducing ventricular outflow tract obstruction on left ventricular hypertrophy and remodeling after alcohol septal ablation. PATIENTS AND METHOD 20 patients with hypertrophic obstructive cardiomyopathy who...
متن کاملParadoxical systolic and diastolic flow abnormalities in hypertrophic cardiomyopathy with mid-cavity systolic obstruction.
Systolic obstruction of the left ventricular outflow tract, either at rest or during provocation, is a frequent finding in patients with hypertrophic cardiomyopathy. Mid-cavity obstruction is uncommon, and intraventricular diastolic pressure gradients in association with either mid-cavity of apical hypertrophy are reported only rarely. We describe a patient with hypertrophic cardiomyopathy with...
متن کاملTransthoracic Doppler echocardiographic analysis of phasic coronary blood flow velocity in hypertrophic cardiomyopathy.
OBJECTIVE To use transthoracic Doppler echocardiography to assess coronary blood flow non-invasively in patients with hypertrophic cardiomyopathy. DESIGN High frequency transthoracic Doppler echocardiography was used to assess resting phasic coronary velocity patterns in patients with hypertrophic cardiomyopathy and to define the relation between coronary flow patterns and clinical, echocardi...
متن کاملSolitary accessory and papillary muscle hypertrophy manifested as dynamic mid-wall obstruction and symptomatic heart failure: diagnostic feasibility by multi-modality imaging
BACKGROUND Solitary papillary muscle (PM) hypertrophy is an unique type of hypertrophic cardiomyopathy (HCM), which is characterized by predominant papillary muscle hypertrophy sparing the rest of other left ventricular segments. It has recently drawn our attention about the mechanism of left ventricular mid-cavity obstruction and the influence of pressure gradient in the left ventricular outfl...
